Responsibilities will include assistance with morning routine (getting out of bed, bathing, bladder/bowel, dressing), meals, bladder emptying every few hours and night routine.  Requires lifting from bed to shower chair, shower chair to wheel chair and wheel chair to bed.  This breaks down to 5-6 hours a day at various times, the largest time commitment is in the morning.  <br>

Qualifications:  Females preferred.  It is vital that the applicant be reliable.  Between 6:30 am and 10 pm, I require assistance every few hours.  I need someone who will be present for every shift and be on time.  If you think you will have other major commitments on the weekends that require many hours, this is not the job for you.<br>

The Room: It is within the Grass Valley city limits (within walking distance of Sierra College). My house is very clean and has been newly remodeled. Renter will have full house privileges. I will cover utilities, including DSL and satellite. Please, no smoking, drinking, drugs, or drama. The attendant, of course, can live here full time; the help I need is just on the weekends.<br>

Compensation:  $500 a month salary plus free room.<br>

<td> The Cottage Facilitator is responsible for interaction between the employee and the child and/or others aimed at preparing the child to analyze and better understand the situation as included in Social Work Activities.  Specifically this includes helping the child to understand the reason for placement and to handle associated emotional problems, resolving the difficulties between child and family that led to the need for placement and planning for the return of the child. <br>
<br><b>Duties include, but are not limited to:</b>

<li> Represent the current treatment and behavioral intervention plan and its intent at provider meetings including Team Decision Making (TDM) Child and Family Team (CFT), Individual Education Plan (IEP), Student Study Team (SST) and any other identified needs. &lt;/li&lt;
<li>Primary point of contact for all residential based services communication with the county placement workers.  </li>
<li>Ensure the cottage integration of skill based and behaviors based programs for the residents. </li>
<li>Assist in specifically training cottage staff to address the individual needs of their residents.   </li>
<li>Coordinate with child’s school and SCH Educational Director to advocate for residents educations rights.  </li>
<li>Coordinate with SCH school enrollment Liaison to assure that residents are in school and appropriately placed. </li>
<li>Serves as a role model and advocate for children and families involved in the wraparound process.  </li><br><br>

<p>- Bachelor's degree in related field preferred.<br>
- Must be licensed in good standing if required by the State Licensing Authority.<br>
- Senior living experience highly preferred.<br>
- Working knowledge of computers including Excel, Word and MS Office.<br>
- Experience with financial reporting and managing multiple budgets.<br>
- Ability to communicate effectively with residents, families, staff, vendors and the general public.<br>
- Must meet all health requirements, including TB, and pass background checks.<br>
- Must have compassion for and desire to work with the elderly.</p>

<p>We are seeking a <strong>Quality and Risk Specialist</strong> to provide oversight of quality improvement activities that focus on wellness, clinical management, medication management, risk management, staff training, education, staff performance and survey preparedness. Specific duties will include managing assigned community to ensure optimal performance, working with the community management staff and corporate staff in planning all aspects of community clinical management, and conducting routine audits of services being provided. Key functions will also include managing the department budget for revenue and expenses, ensuring optimum occupancy and resident retention through sales and marketing efforts, and making certain that staff members are providing excellent customer service to our residents. Other duties will include ensuring that employees receive required training.</p>

Minimum Eligibility Requirements:<br>
- Licensed Registered Nurse<br>
- Must be licensed in good standing if required by the State Licensing Authority<br>
- Proven record of success as a Nursing Director in Assisted Living or Long Term Care environment.<br>
- Must be able to communicate effectively with residents, families, staff, community officials, state regulatory agencies, hospitals and the general public.<br>
- Must meet all state health requirements<br>
- Subject to criminal background check<br>
- Must be able to perform duties and responsibilities with or without reasonable accommodation<br>
- Must be able to work within a constantly changing environment.<br>
- Must have at least two (2) years of management experience in the Assisted Living or Long Term Care profession. <br>
- Ability to work full-time, with some weekend work.<br>
- Availability by phone 24 hours per day, 7 days per week.<br>
- Ability to travel to assigned community (up to 100%) without restriction.
