Sacramento Self-Help Housing participates in the plan to end chronic homelessness in Sacramento by housing people right off the river in a "housing first" program using leased homes with on site house monitors. The Case Worker will work with 40 formerly chroniclly homeless residents living in homes at scattered sites throughout the city of Sacramento facilitating group living, group dynamics and living skills training. Individualized case management with wrap-around services and support for community activities are offered to all residents. Assistance with immedate needs such as obtaining food, clothing or medical attention. Harm reduction stratagies. Mental health and AOD assessment, referrals and relapse prevention. Crisis intervention.
Qualifications include: BA/BS degree in related field or at least two years of related work experience. Experience working in human services, particularly with homeless/disabled people. An ability to work independently and as a part of a team. Extensive local travel is required, so the applicant must have reliable personal transportation.
